RE: REQUIRED 2018-19 HEALTH FORMS AND AUTHORIZATIONS
 
Please read the following carefully. For physicals scheduled after July 15, see the first page in the packets to be printed out below.
 
IMPORTANT NOTE: Students will NOT BE ALLOWED to participate in pre-season athletics or attend school until all required forms are received and recorded by the Nurse’s Office.
 
THIS IS A TWO-STEP PROCESS FOR EACH CHILD:
 
1. ELECTRONIC FORMS:
In the yellow bar at the top you will see "Forms to Review." Click there and complete online as follows: 

   a.  2018-19 School Activities Permission Form
   b.  2018-19 Medical Information (you will need physician contact and insurance information).
   c.  2018-19 Notification Settings (Emergency Contact Information)
  
2.  PAPER FORMS:
PRINT OUT FROM LINKS BELOW.  ALL FORMS MUST HAVE REQUIRED PARENT AND/OR PHYSICIAN SIGNATURES.
 
*Please disregard forms that may not pertain to your child, ie. Asthma Plan, Allergy/Anaphylaxis Emergency Care Plan.
 
  • All forms necessary by grade level are included in the links below.
  • Please click on your child’s grade for 2018-19 and print out to review and take to your physician.
