Athletic Hall of Fame Nomination Form

Please complete this form to nominate alumni, a team, supporter, or a coach to the Dwight-Englewood Athletic Hall of Fame.  Nominations are accepted at all times.  However, for consideration for the Spring ceremony, nominations are due by October 15 the prior year.  Because we have had to postpone the last two year's Athletic Hall of Fame induction ceremonies, we will not be honoring any new alumni athletes, teams, coaches, or supporters at the 2023 ceremony.
