Health & Medical Forms Reminder:

Important Please Note:  All Middle / Upper School students who wish to play on a D-E Athletics Sports Team must have submitted current Health & Medical Forms to the School. Enrolled D-E families should have received reminder information by Email directly from the Magnus Health Portal, about completing all these required Health & Medical forms.

To access the Magnus Health Portal, D-E families must LogIn to MyD-E. After you LogIn to MyD-E go to the Parent Resource Board for the Magnus Health Portal link (see above visual.)

Need help with MyD-E LogIn? Please contact D-E's Technology Department at help@d-e.org or by phone: 201-227-3177.
D-E Concussion Protocol:
If an athlete suffers a trauma to the head, or a blow to the body, resulting in their exhibiting symptoms of concussion, then the athlete will be removed from the game/practice. The athlete will then be referred to a physician who is trained in concussion management, for authorization to either resume immediate participation or begin a “return to play” protocol. Questions may be directed to the D-E Upper School Nurse at USNurse@d-e.org or Kim Conover (D-E Athletic Trainer) at: conovk@d-e.org.

Statement RE: NJSIAA Steroid Testing (For Upper School Athletes Only)
NJSIAA steroid testing policy states that if an Upper School student athlete or students team qualifies for a state championship competition, the student may be subject to testing for banned substances. Questions may be directed to the Athletics Director at: schmic@d-e.org.
